Jetstream ZS-NRM

The Flight Safety Group has completed a comprehensive accident investigation report, requested by a relative of one of the accident crew members and in response to the South African Civil Aviation Authority (SACAA) final accident investigation report REF: CA18/2/3/8690 - detailing the Jetstream Aircraft 4100 ZS-NRM, loss of control, following an engine failure and purported misidentified engine shutdown after take-off from Durban Airport, in South Africa on 24th September 2009.

This comprehensive accident investigation report follows the application of the Flight Safety integrated Tripod - HFACS/HFIX accident investigation methodology technique – which forms an integral component of our Accident/Incident Investigation Awareness (AIIA) training course, delivered in a face-to-face classroom environment, or online as live-learning, in a virtual classroom environment.

The Jetstream accident analysis report highlights the human factors, fallible decisions and latent failures involved in this accident, and refutes the misguided conclusions in the SACAA report.
